About the Foundation The Crossroads Hospice Charitable Foundation is passionate about promoting and supporting end-of-life care and services through the granting of financial resources to enhance community education and awareness. The Foundation supports the programs and activities of community and non-profit organizations like: educational seminars, caregiver tools, fundraising activities and more. Each request is considered on a case-by-case basis. To be considered for, and ultimately receive funding, all interested parties must complete the “Request for Funding” form completely and accurately and adhere to all guidelines set forth. Request for Funding Form: The “Request for Funding” form contains all of the instructions your organization will need to successfully complete the form. It is advisable to print a PDF of the form, pencil in all of the information and then complete and submit the online Request for Funding form.
